Your RAG system just gave a wrong answer. Where do you look first? Most teams blame the prompt. They're usually wrong. The real culprit is retrieval. And retrieval traces back to something upstream that rarely gets attention: how well your documents were prepared in the first place. When a RAG system fails, it almost always falls into one of three modes: > Relevance failures: it pulls the wrong content > Coverage failures: it misses content it should have found > Noise failures: it drags in clutter that muddies the answer All three trace back to ingestion quality, not prompting. When ingestion doesn't preserve structure, tables lose their column headers. Exception clauses separate from the rules they modify. The information needed to answer a query never becomes a coherent, retrievable unit. You can rewrite prompts forever. But if the corpus was built badly, the answers stay unreliable. So, what actually works? Test retrieval as its own component. Then trace every failure back to where the corpus was built. That's the difference between AI that works and AI you merely hope works.
