Medicare part D pays $27 for one capsule of Omeprazole??? No independent pharmacy has ever been paid more than $10 for 30 Omeprazole. The average is around $3 for 30. That's a lot of spread and government waste. I fucking hate this system.

The imaging center confirmed insurance approved it, and his copay would be four hundred and something dollars. Then he asked the obvious follow-up. What if I just pay cash? A few clicks later: $250. Cheaper to leave the insurance card in his wallet. On a scan his insurance had already approved. This isn't rare. It's the pattern every time somebody actually checks. Medicare pays about $475 for an MRI at an independent imaging center. Cash prices at freestanding centers commonly run $400 to $1,200. Run the same scan through a hospital outpatient department and it can land anywhere from $1,500 to $3,500, plus a facility fee. A Johns Hopkins study published this year compared hospitals' posted cash prices with their insurer-negotiated prices for the 20 drugs Medicare spends the most on. At up to half of hospitals, depending on the drug, cash beat the typical negotiated rate. At up to 81%, cash beat the highest one. The researchers' explanation was simple. Providers like getting paid up front with no claims, no denials and no waiting, so they price cash competitively. Same story at the pharmacy counter. Another Hopkins study found Medicare Part D paid $27.85 a pill for omeprazole that costs about 80 cents over the counter. Across 19 common drugs available both ways, Medicare could have saved $406 million in one year just by paying the cash price. So why does the insured price come out higher? Because a copay isn't a price. It's a number your plan sets based on its benefit design, and it has nothing to do with what the scan or the drug actually costs. Meanwhile the provider has to file a claim, wait on it, fight denials and pay staff to chase it. All of that cost lands in the insured price. Cash skips every step. Instead, we're pretending we can grow our way out of this with AI productivity gains creating $50 trillion of value in the next ten years.
With the interest rates so high, the next few decades are going to really amp up the level of political confrontation. No one wants to be left holding the bag for servicing all that interest. So different groups will fight very hard to pin it on someone else. Right now boomers seem to want to foist it off on young workers but that isn’t going to work for much longer anymore.

What can you purchase at a gas station for less than a dollar? What can you purchase at a “dollar store” for less than a dollar? Hell, what can you get ANYWHERE for less than a dollar? Unheard of, right? Then someone explain to me why it is acceptable (and common practice) for many insurance companies to pay pharmacies less than a dollar to dispense a prescription. Not even including overhead costs like professional licenses like state board and DEA, liability insurance, internet access, software subscriptions, etc.. The majority of pharmacies are already starting underwater before even touching the prescription to get it ready for the patient. People don’t hate their insurance company enough. Whatever is coming down the pipeline from a legislative standpoint is only just scratching the surface of their problematic business practices.

The Tax System Explained in Beer Suppose that every day, ten men go out for beer and the bill for all ten comes to $100. If they paid their bill the way we pay our taxes, it would go something like this: - The first four men (the poorest) would pay nothing. - The fifth would pay $1. - The sixth would pay $3. - The seventh would pay $7. - The eighth would pay $12. - The ninth would pay $18. - The tenth man (the richest) would pay $59. So, that's what they decided to do. The ten men drank in the bar every day and seemed quite happy with the arrangement, until one day, the owner threw them a curve ball.  "Since you are all such good customers," he said, "I'm going to reduce the cost of your daily beer by $20".  Drinks for the ten men would now cost just $80. The group still wanted to pay their bill the way we pay our taxes, so the first four men were unaffected.  They would still drink for free.  But what about the other six men?  How could they divide the $20 windfall so that everyone would get his fair share? They realized that $20 divided by six is $3.33.  But if they subtracted that from everybody's share, then the fifth man and the sixth man would each end up being paid to drink his beer. So, the bar owner suggested that it would be fair to reduce each man's bill by a higher percentage the poorer he was, to follow the principle of the tax system they had been using, and he proceeded to work out the amounts he suggested that each should now pay. And so the fifth man, like the first four, now paid nothing (100% saving). The sixth now paid $2 instead of $3... (33% saving). The seventh now paid $5 instead of $7... (28% saving). Each of the six was better off than before.  And the first four continued to drink for free. But, once outside the bar, the men began to compare their savings. "I only got a dollar out of the $20 saving," declared the sixth man.  He pointed to the tenth man, "but he got $10!" "Yeah, that's right," exclaimed the fifth man. "I only saved a dollar too.  It's unfair that he received ten times more benefit than me!" Assimilation is mostly a myth, because people are not blank slates. We can identify personality factors that have varying degrees of heritability (never zero). And there are almost certainly differences on these measures between groups. The pro-immigration people should just bite the bullet and admit that immigration irrevocably changes the character of the nation — but that’s good! The idea that everybody ultimately blends in together as the immigrants acclimate to our way of life is basically just absurd.

Making America worse by importing inferior foreign culture is bad. Making Americans pay for the privilege of watching their country decline is bad. Replacing Americans in their own country is bad, because we are a people and this is our home and reducing our heritage and birthright to a mere global economic zone is bad. Working to eradicate Americans and American culture is bad. Opposing the takeover of your own country and the degradation of the very culture that made your country the greatest country in the history is not only not racist, not xenophobic, not-whatever-dumb-gay-term-is-now-being-used-to-morally-extort-you-into-giving-away-your-country—it is the only natural response to what is being done to America. Foreign culture is inferior to American culture, and we are all sick and tired of constantly being subjected to it while being told it’s not happening.
