afib w/ rapid ventricular response and right axis deviation; assess stability of bp and consider shock; V1 has some artifact. no WPW currently present IMO chest xray, echocardiogram and troponin/BNP and begin appropriate beta blockade; do not give non-dhp CCB because of HF risk
