Jamii ni Afya (Zanzibar) has achieved national scale as one of the world's first government-led digital community health programmes, with 95% of referrals by Community Health Workers successfully reaching health facilities and 86% of women delivering in health facilities.

Afya-Tek (Tanzania) is connecting Community Health Workers, health facilities, and private community drug shops through a digital platform. It has achieved a 91% successful referral rate and improved treatment accuracy, setting the foundation for sustained national scale.
