We keep looking for the bad actor. The deeper problem is the incentive structure. If capability is rewarded and restraint is individually punished, rational actors will keep pushing even when they genuinely believe the system is dangerous. That's not a morality problem. It's a coordination problem. At some point, “please be careful” stops being a safety mechanism. You need everyone to stop at the same time.
