60 y/o with unknown thrombophilia on AC since 1990's. He was admitted to our sister hosp 3 weeks ago with syncope, extreme pain, and severe bilateral swelling. He went off DOACs 2 months prior due to cost. Had also prior Optease in 2017 for unclear indication. An adventure begins

Patient reports resolution of pain and LE swelling. Patient refusing enoxaparin so back on DOAC. Expensive and time consuming case but good outcome. Multimodality approach is sometimes key with mixed morphology clot. @NCVHonline @VIVAPhysicians @ISETNews @_backtable @WLGore
