My MD Psychiatry thesis was with teenagers living with unmedicated ADHD, and as someone who has worked with >100s of such kids, and eventually plenty adults to form an opinion, let me tell you my experience as a doctor, therapist, and observer of over 3 years of ADHD meds: 1) adults who were struggling in their drop years for JEE and NEET, had extremely low self esteem for being top scorers during school but failures in such exams, finally made it to government/private medical-engineering colleges (NO, they did not suddenly get AIR 1) 2) adults who grew up with undiagnosed and unmedicated ADHD now presented with moderate-severe personality disorder diagnosis. Medicating them FOR ADHD helped lessen the symptoms of personality disorder without using antipsychotics or mood stabilisers (especially borderline personality disorder AND self harm) 3) smoother romantic relationships for the adults on medications, after consistently feeling that they ruined every relationship they touched, they finally had something stable, and better relationship with food too (even though its well known that methylphenidate causes appetite loss- these people were now ATLEAST able to remember to have food timely, meal prep, calorie prep, etc.). Consistency>chaos FINALLY 4) teenagers were finally NOT getting beaten up at school and at home for leaving homework unfinished or for causing chaos or for running around in class 5) their C and D grades were finally jumping off to Bs. A few subjects had As too but I wouldnt want to exaggerate. The parents however, would often thank me and cry in the OPD. 6) someone who could barely get 50% marks eventually got >80% in 10th boards with 2 years of treatment and therapy! 7) cigarette use, alcohol use, marijuana use came down! I could go on, the differences with and without medications were life changing for ALMOST ALL. Addictive substances on the other hand RUINS academics, relationships, self, and everything in between. I cannot say I am addicted to food. Even though since I was born, I have been developing tolerance to how much joy i get from tasty food, after 4-5 hours I start feeling hungry for food (withdrawals), and everytime I watch someone eat a delicious meal, I too want to have it (craving). Sometimes I eat food in the middle of a hectic OPD or class (loss of control). I also enjoy icecreams when I have a sore throat despite it worsening my health (use despite harm). And many days, I cannot stop thinking about what tasty food I will eat once I reach home, there have been instances when I go get food in the middle of work (salience). According to ICD-10 criteria, I should be addicted to food. MAKE IT MAKE SENSE. YOU CANNOT.
Concerta is so addictive I missed one routine pharmacy run and didn’t go pick it up for so many months I had to go get a new prescription. You ever met a meth addict who forgot to pick up readily available insurance covered meth for several months? Like meth users are so busy being a working wife and mom they forgot to use meth. Clowns.

Yesterday, I went to play Garba at a place 15 kms from my house, where Rapido wasn’t working at the time. So I made my father wait outside the hall in case I get a seriously ill referral patient from nearby areas, so that I can leave quickly and arrange ICU care. After 5 minutes of waiting, both of us realised I don’t not know much ICU work, and I am also not employed in an ICU. He went home and slept peacefully, and I enjoyed 3 hours worth of Garba.

A good bechmark i followed for whether a topic/incorrect mcq went into my chhoti copy or not was looking at the % of people who had marked the correct answer. If it was >20%, i would add it. Anything under it, I would avoid adding it and MOVE ON assuming some crazy quizzer mustve added that mcq to the GT at Marrow HQ and wont necessarily change my chances of getting a rank since >80% people either did not know the answer/did not cheat with google/did not pick the correct guess-option. Ultimately, it was always me and my chhoti copy against the MCQ Gods and I cant take any slander against it 😓
Chhoti copy is a topper thing. Don’t take it the wrong way. Most people can’t make a proper CC. It takes time and a good understanding of your weaknesses and what’s actually important. An average student will add everything important to their CC, which defeats the whole purpose.

This probably wouldnt work anymore because kids have told me people are using AI to cheat during GTs 🤡 hence ruining the % game.

What do you expect a GP to do? Help you get a Job? Talk to your father that abused you ? Help you study for exams? Their options are refer to therapy and medication (after considering other medical causes)
Is it just me or do GPs seem to be handing out sertraline like candy, for someone who is stressed What about trying to resolve the route cause

Re: BHMS doctor "saves" life of ICU patient in Gujarat- This whole charade appears to be a badly staged publicity stunt for the hospital this couple runs. Look at the ventilator for e.g. no actual patient waveform looks like that. It seems to be on demo mode screen! @rjVACHAS

Often the self-proclaimed “self-aware” people use rationalization and psychobabble as an excuse for genuine insight. Everyone has blind spots when looking inward. That’s why therapists go to therapy.
does therapy even work when you’re self-aware enough to see through your own bullshit and the therapist’s, so the whole session just feels like being gaslit by someone with a psychology degree 😭

"It is difficult to recognize a pattern when you're immersed in it" – David Olds on 'decentering' for insight. For example, we thought with reasonable confidence that the earth is the centre of the solar system. Some geniuses could work out the truth by complex calculations – and still many would doubt it. But even an average individual who is in a 'decentered' position outside the solar system could see the true pattern in an instant!
1) you are self aware. You have found reasonable reasons as to why you behave or feel a certain way in certain situations 2) yet you are miserable. If you werent- would you need to look up therapy? You wouldnt! 3) if you know your patterns, if youre reasonably happy and satisfied with your life, if youre functioning well, why do you need a therapist? I wouldnt spend on an asthma pump if i DONT have asthma! 4) If you ARE NOT well- then clearly there are certain things you can be helped with. By a therapist, a psychiatrist, a coach, a psychosocial worker, a friend. In a random order of severity and needs.

there are many people who are too self aware for most therapy methods. if you make tweets like this 9 times out of 10 you aren’t one of those people.

I genuinely don’t get the concept of Internal Bleeding. Are we not supposed to bleed inside? Is blood not supposed to be inside the body? So what exactly do you mean by Internal Bleeding, and the fact that it’s even life-threatening confuses me a lot. Like, isn’t blood supposed to flow inside the body? 😬

“I am clearly so much smarter than the therapist heh! 😏” “The therapist looks AND sounds stupid 😏” Meanwhile your life is falling apart and you still refuse to acknowledge that its YOU who is responsible for that. You refuse to acknowledge that your IQ has not saved you from unhappiness, loneliness, or disappointment. Or any of the xyz problems you may have. The person sitting on the couch need not be “stupider” than the therapist- just willing to explore and find solutions together.
therapy only works if you have a lower IQ than the therapist
