Before Obamacare every medical facility had a singular billing department that collected and organized patient billing into a single statement.

It was worse before Obamacare, and Obamacare was woefully inadequate. One of the big reasons for this is Obamacare was a copy/paste of RomneyCare which was created by the Heritage Foundation. If you hate ObamaCare, thank the Right Wing thinktank.
