The latest NCCN Breast Cancer Screening & Diagnosis Guidelines (v1.2026) includes a pivotal update: image‑based risk assessment is now recognized as a valid way to identify women at increased risk of invasive breast cancer. Image-based risk is a 5-year breast cancer risk assessment approach that can be derived from routine, negative, screening mammograms. By recognizing this as complementary to traditional clinical and hereditary risk models, the NCCN guidelines introduce a new pathway of visibility into an individual’s risk of breast cancer. With this update, clinicians have a new option to support patients identified as “increased risk” with: ✅ Tailored screening strategies ✅ Additional surveillance ✅ Ongoing risk reassessment ✅ Consideration of counseling for risk reduction strategies At Lunit, we see this as an important step toward more accessible, image-driven breast cancer risk assessment. It also underscores the importance of innovations like Lunit INSIGHT Image-Based Risk – a solution designed to estimate a woman’s 5-year breast cancer risk using routine mammographic images and age. There's something about a room full of people who have dedicated their careers to breast cancer care that puts things in perspective. This year felt especially meaningful with the conference celebrating its 35th anniversary, a testament to how far the field has come and how much further this community is committed to taking it. Brian Drohan, PhD, Medical Science Liaison at Lunit, opened Monday morning with a session on risk assessment in contemporary practice. He walked through where science is today and where it's headed. Screening is more than early detection. It improves survival in stage IV breast cancer. For years, mammography has been discussed mainly as a way to detect cancer early. New research in the Journal of the National Cancer Institute shows its impact reaches much further than that. A study of more than 817,000 women in Denmark found a dramatic difference in five‑year net survival for women diagnosed with Stage IV breast cancer: → Screen-detected: 74.7% → Never-screened: 32.4% This gap is not explained by lead‑time bias. Screen‑detected Stage IV cancers were more likely to be oligometastatic and far more likely to be treated surgically. 67% of screen‑detected cases were candidates for surgery, compared with 23% percent in women who were never screened. Screening doesn’t just find cancer earlier. It changes the clinical trajectory of advanced disease and expands what is treatable, even at Stage IV. As pressure on radiologists and technologists continues to increase, AI plays a growing role in supporting accuracy and consistency.
