A KLM flight from Amsterdam to Curaçao was forced to turn around over the Atlantic on Thursday after a power bank fire. The power bank was being used by KLM´s former CEO and President, Mr. Camiel Eurlings, who was injured in the incident. "According to a passenger who was sitting close to Eurlings and saw everything happen, the power bank was charging at the moment things went wrong . Power banks are allowed on board KLM flights, but it is not permitted to use or charge them on board." Incident occurred on a flight from Amsterdam to Curacao. Today, a man invaded Guarulhos Airport, Brazil´s largest airport. Around 2:20 p.m., the man crossed physical barriers and walked to runway threshold 28R, opposite threshold 10L, which was being used by a aircraft for takeoff. Exclusive footage obtained by the GolfOscarRomeo channel (link on the post below) an AEROIN partner, shows the man standing on the runway as a LATAM Airbus A320neo departs for Porto Velho on flight LA3568. The aircraft passes at a considerable distance from the man, since runway 10L/28R is 3,700 meters long and the jet became airborne in the first third of the runway. piped.video/BjkvG_4rJNU?si=bmqh…

A bit like on aircraft, a fire aboard a boat is one of the most dangerous emergencies. It can spread rapidly due to fuel, electrics and confined spaces, leaving little time to react. And there is no easy way to escape. (jumping on the sea is easier said than done)
WATCH: A big fire broke out on the Greek-flagged car ferry "Blue Car Carrier II" north of the island of Mykonos. The vessel had 29 people on board and nearly 200 trucks and cars, no injuries were reported.

Lethal turn - Indonesia´s deadliest air disaster was OTD September 25, 1997: Garuda Indonesia Flight 152, an A300, crashes in Medan (Indonesia); all 234 aboard die. On approach, jet flew under assigned altitude of 2000 ft, crashing at 1550 ft. ATC error, poor visibility, crew lack of awareness and GPWS issues were some of the factors. Inquiry highlighted crew confusion over a turn — a left turn issued instead of a right turn at a critical point, under radar vectoring — shifted the crew’s focus to lateral navigation and reduced their awareness of altitude, continuing the descent below minimum altitude. The report also criticized the airline’s conversion training for pilots who flew both the A300-600 (digital navigation displays) and the A300B4-FF (analog instruments): the captain’s limited familiarity with analog instrumentation may have left him overloaded. Contributing factors included failure of the Ground Proximity Warning System (GPWS) for undetermined reasons and inadequate vectoring charts used by Medan controllers. In fact in the RAF, at one stage, all the pilots were told to wear a hi vis vest as they walked to their jets. It was in case they got hit by a car. There were no cars. And they we all went and flew down valleys at 250 ft, 500 mph in the dark.
"You should be wearing hi-vis."

I´m a skeptic of paranormal, but sometimes I think the F-104 was jinxed A few months ago, I twitted on an accident involving four Luftwaffe F-104s before entering service. Now in another eerie coincidence, years later the Italian Air Force also had four F-104s crash in similar circumstances. September 25 1975: A formation of four Italian F-104´s crash in Godendorf (West Germany); all 4 pilots died. The jets were part of a NATO unit and crashed near Bietburg, a US Air Force Base. The control tower operator stated radio contact was lost almost immediately after the flight took off into overcast skies. The registrations of the aircraft involved were the following: MM6516, MM6575, MM6508, MM6523. Video is from a Italian TV series (don´t know the name), depicting an F-104 scramble, kindly uploaded by YouTube user Ádám Papp* (*Added his watermark, so other pages don´t download the video, upload it as their own and add THEIR OWN watermark on IG, as it has happened before. That is disrespectful AF: share, OK, but give credit, otherwise its stealing others work)

Some occurrences from this date (September 25 – more will be tweeted throughout the day) ➡️ 2020: a Ukrainian AF An-26 [”76 yellow”] on a training flight crashes in Chuhuiv (Ukraine). 26 of 27 aboard die. On approach, plane went out of control, hit ground 0.8 mi from runway. Factors: engine failure, breach of flight schedule and rules, crew actions on emergency. ➡️1998: Paukn Air Flight 4101, a BAE 146, crashes in Boumahfouda (Morocco). All 38 aboard die. Jet impacted high terrain on approach to Mellila. Investigation stated crew descended below safe altitude, had inadequate coordination and did not react correctly to GPWS alarm. ➡️1996: a private DC-3 crashes off Den Oever (Netherlands). All 32 aboard die. Crew reported engine issues before losing control and going down in a mudflat. Factors: mechanical problems, crew actions (CRM, training, emergency actions), others. Aircraft was operated by the “Dutch Dakota Association” [Reg. “PH-DDA”] ➡️ 2011: Buddha Air Flight 103, a Beechcraft 1900, crashes in Kathmandu (Nepal). All 19 aboard die. On a sightseeing tour interrupted by bad weather, plane made CFIT. Factors: crew actions (captain fatigue, inexperienced F/O, poor CRM); lack of TAWS. ➡️1946: a RAF C-47 [Registration “KN414”] crashes near Kai Tak airport (Hong Kong). All 19 aboard die. Shortly after take-off, aircraft reached around 3000 ft before going out of control. It´s likely the plane stalled after encountering turbulence from the foothills.

Textbook emergency landing after an engine failure at only 600 ft from the ground. They had 40 seconds to react, and they saved the day: no one was hurt. This one goes viral from time to time, but it's an "oldie" and with a sad note in the end On 26 May 2020, this aircraft (I could not confirm the type, but I think it was a light seaplane, an “Aero Adventure Aventura II”) took off on a routine flight from Williamsburg-Jamestown to Connecticut when it experienced this emergency. FAA inspectors later confirmed the loss of fuel pressure was due to a cracked fuel rail, and the incident was not due to pilot error. The aircraft was operated by Mr. Antony Yen, a flight instructor with a company named “Straight and Level Seaplanes” (I could not confirm if this aircraft in particular was operated by the same company; in fact, I couldn´t even find if it was written off in the occurrence) So, what´s the sad note? In August 13, 2022 an Aero Adventure Aventura II on a test flight crashed in Miami, killing both occupants. One of them was Antony Roland Yen. The cause was traced to a maintenance issue.

San Diego Mid-Air Collision was 48 years ago September 25, 1978: PSA Flight 182 (Boeing 727) and a private Cessna 172 collide over S. Diego, California. All 137 on both planes and 7 on the ground die. PSA crew did not comply "with the provisions of a maintain-visual-separation clearance”. ATC issues were also a factor. More details below ⬇️ Inquiry stated that the airliner’s crew failed to maintain the visual separation they had been assigned and did not tell controllers they had lost sight of the small plane. Air traffic control procedures at the time also allowed visual “see-and-avoid” separation in busy terminal airspace even though radar could have provided positive lateral or vertical separation. Contributing factors included the Cessna turning off its assigned 070° heading without notifying ATC, the approach controller not informing Flight 182 which way the Cessna was moving, and the controller’s failure to act on a conflict-alert warning that sounded seconds before impact. (which was also known to provide “false alarms” due to its unreliability). These combined errors left the descending jet overtaking the climbing Cessna until they collided. Video is from the Weather Channel TV show “Why planes crash”

This was in February 2024. All aboard survived, but regardless, what a scare it must have been!
This terrifying spin happened only seconds after this Bell 206L-3 lifted off from a rooftop helipad in Medellín, Colombia. Six people were inside the helicopter as it spun violently just seconds after takeoff, slammed into the building and ended up suspended from a structure. From the footage, it looks like the kind of accident where the outcome could have been completely different. There was almost no altitude, no time to recover, and a high-rise building directly beneath the flight path. Seems to me like a helicopter tail rotor failure, but there could be several other possibilities, including the engine, flight controls, wind and other environmental conditions, as well as the takeoff technique. Watching those few seconds shows just how quickly a helicopter can go from a normal takeoff to a loss-of-control emergency!

Continuing to showcase his flexibility skills, the latest @MenTourPilot video disaster documentary moves from aviation to one of the worst industrial disasters in history: the Bhopal tragedy if 1984. Worth a look as always (especially for the people working on engineering)
On 3 December 1984, over 500,000 people in the vicinity were exposed to the highly toxic gas known as methyl isocyanate which is almost as devastating as cyanide, this would layer to become known as one of the worst chemical disasteres ever... 👇 piped.video/Z_WGhCl9S4I
