HSIL (AIN3) with striking replacement of anorectal crypt epithelium, producing multiple rounded dysplastic crypt profiles on low power.
The appearance brought to mind cervical CIN3 with gland/crypt involvement.
Miliary lung disease occurs when a massive number of pathogens break out of an initial infection site, travel through the bloodstream, and blanket the lungs with thousands of tiny, millet-seed-sized spots
This is miliary tuberculosis, but the same principle applies to disseminated histoplasmosis and coccidioidomycosis

In this fixed cross section, you can see the mass off the surface on both sides of the tibia, as well as the small adjacent satellite nodule within the soft tissue.
This mammary myofibroblastoma arose in a man's breast (this lesion is more common in men than women). It co-expressed desmin and CD34; Rb protein loss would be anticipated! Good bread and butter!

An adenomatous polyp with any amount of TSA component should be classified as TSA. If it has high grade dysplasia, then call it TSA with high grade dysplasia
