You leave the TV on to fall asleep. It feels like rest, but the room is not dark. Light at night has been linked with poorer glucose control and metabolic health. Eight hours with a bright screen in the room is not quite the same as eight hours in darkness. Turn the TV off. If you need a light, keep it dim, warm and low.

BPH can contribute to infertility in some men, primarily by impairing ejaculation rather than sperm production. Severe enlargement or associated inflammation may reduce semen volume or, rarely, obstruct the ejaculatory ducts. Treatments pose a greater risk sometimes : alpha-blockers and 5-ARIs often cause reduced or retrograde ejaculation, while surgery such as TURP frequently produces permanent dry orgasm. “The standard of care interview during a BPH consultation needs to counsel men on therapies that address urinary symptoms and preserve ejaculation. Because once it’s lost, it’s lost. Many men with milder BPH still conceive naturally. Semen analysis and fertility-focused treatment selection are advised when paternity is desired.​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​ #BPH

A young woman broke down in tears in the clinic. Not because she had a terminal illness. Because she had lost hearing. She had been treating herself with medication that can damage the ear. It was just after morning ward round. A young man stopped me near the exit of the clinic. “Doctor, can I ask something small?” He looked healthy. Early 30s. Clean shirt. Office worker type. He said he only came because his company insisted on a routine medical check. “No problem at all,” I said. Then he leaned closer and lowered his voice. “Doctor… is it possible for someone to have high blood pressure and not know?” I nodded. “Very possible.” He laughed. “Ah. But I feel perfectly fine.” I wasn't taken aback because I hear that sentence almost every now and again. So I asked him a simple question. “When was the last time you actually checked your blood pressure?” He paused. Thought about it. Then smiled a little. “Doctor… maybe during NYSC.” That was almost 5 years ago. We checked it there in the clinic. The Sphyg beeped. He looked at the screen. Then looked at me. 190/110 mmHg. The smile disappeared from his face. “Doctor… but I’m not sick.” I responded that “that is the tricky thing about high blood pressure.” Most times, it doesn’t knock loudly. No pain. No warning. It quietly strains the heart, the brain, the kidneys… for years. Until one day the body finally protests. A stroke. Heart failure. Kidney damage. All from something that could have been detected in two minutes. That morning he didn’t leave with medications alone. He left with something more important. Awareness. Sometimes the most dangerous illnesses are the ones that feel perfectly normal. If you haven’t checked your blood pressure in a while, today might be a good day. It takes two minutes.
