Try version 1.14.0 - holding option turns the "Listen To" menu into "Don't Listen To" so you can exclude apps while listening to system audio.

Hey, you can choose to listen to a specific app already. Or press ⌥⌘S or the Pause subtitles option in the menu to temporarily disable subtitles. Then press again to re enable them.
