I could be running a $5K/month med spa retainer right now, taking on 30 clients, promising them the moon, and delivering mediocre results because the budgets don't cover real work. Instead I only work with brands willing to dump $30K+/month into ads with a real infrastructure play behind it. Fewer clients. Better results. Actual case studies. We shall see who wins the race in a few years time.

Most telehealth agencies fail because the people behind them didn't care enough. Literally all you have to do to succeed in this space is: ⦁ Take on clients you can actually make money for ⦁ Refuse the ones you can't ⦁ Deliver until they hit their number ⦁ Refer out the rest That's not talent. That's not genius. That's brute force. Go.

Just got a testimonial from the CEO of E-Mortgage Capital (one of my biggest client). He called my strategies "cutting edge, innovative, outside the box." Then said what matters most: "Good guy, good heart, man of faith." Big W. That’s why we do what we do.

"Why do you work on the sales team so hard?" Because leads mean nothing without a closer. I've watched med spas spend $50K/month on ads, generate hundreds of leads, and convert 3% because nobody trained the sales team. Meanwhile the clinic doing $250K/month with $49 offers hires salespeople, puts them through a $6K training program, and dresses them in lab coats. Same leads. Wildly different revenue.

In telehealth, you could build a $10M/year brand in 12 months. Or you could get locked in a broken white label for 12 months and lose $500K on setup fees, unprocessed refunds, and missed market timing. Both the most opportunistic time in telehealth history AND the most unstable. Pick your white label wrong and you never see the upside.

The entire theme of telehealth is: Solve one boring medical problem for one specific patient and get paid recurring revenue for years. Nobody in this space is inventing new molecules. You're just serving trainer clients at busy gyms, postpartum moms in Newport Beach, aging execs who want their edge back, or biohackers stacking peptides. Small niche. Big check. Nobody outside this space would understand how good the math is.

How to build a telehealth brand in the Andromeda era, step by step: 1. Draw the funnel on a whiteboard (ICP, offer, intake, cross-sells) 2. Pick ONE avatar to dominate first (menopausal women, executive TRT, postpartum, etc.) 3. File LegitScript in week 1 (4-8 weeks to approve) 4. Consolidate into ONE Meta campaign with 15-20 diverse creatives 5. Feed the AI. Stop trying to outsmart it. 6. Optimize for lifetime gross profit, not first-week ROAS Founders skip step 6 and wonder why they cap at $200K/month. Don't be that founder.

I wish I could pay you $1,000 to read this. Because this breaks down the single decision that determines whether your telehealth brand becomes a real business or a $30K disaster. (Hint: 93% of failed telehealth brands got this one thing wrong.) If you're launching in 2026, this will save you 6 months and six figures 🧵

My top-performing telehealth quiz questions were always: ⦁ Emotionally loaded ("What would reaching your goal mean for you?") ⦁ Educational ("82% of Americans aren't the weight they'd like to be") ⦁ Specific ("Has anyone noticed or commented on your hair loss?") ⦁ Framed as care, not qualification Don't overthink it. Ask patients questions their own doctor would ask if they had 30 minutes instead of 6.

THE BOTTOM LINE: Your white label decision is the single highest-leverage move you'll make in the first 90 days. Get it right and everything else compounds. Get it wrong and you'll spend 12 months learning why the shortcut didn't work. Same market. Same opportunity. Different foundation.
