Real recognizes Real. Many have been fighting the good fight that enucleation is the gold standard for years. Few have been advocates as long as Javier. Grateful to call him a friend and like minded urologist. @SocietyofBPD @Endo_Society
20 years doing #enucleation, more than 5000 patients, more than 30 countries, #AEE #holep. I still remember when others doing green light vaporización were saying enucleation was impossible!

As the @IFRUEUrol scientific chair and @SocietyGURS fellowship director, putting this session together with young talents like @BrianInouyeMDis what it's all about. Fantastic job breaking down DCB dilation and bringing the fire to the stricture crossfire! Brian, you have done outstanding work championing our early-career faculty and driving forward educational initiatives for young urologists across GURS and beyond—it’s making a tangible impact on the field. Thrilled to have had you collaborate with us at #IFRUE2026. Safe travels, and looking forward to catching up in November in Mexico City! 👏🔥 @NickelCityUro @boyd_viersMD @ZLeeGU @kdoersch689

This convo happened years ago, but I was reminded of it recently. Me: ”What do I do when I have a trainee who just fucking gets it? They understand surgery like breathing. How do I teach them when they have that level of innate skill?” Mentor: “There is only one thing to do. Enjoy. And then watch them soar.” Me: 🤔 ❤️

As new therapies for BPH continue to emerge, we often ask where might these treatments play a role 🧩. The new generation of prostate stents are not a replacement for surgery, but may be better options than medical therapy in certain settings or than Urolift. Two recent manuscripts in Urology highlight clinical scenarios where current BPH treatments reveal gaps that this new generation of reversible nitinol prostatic stents may help fill. Open letter to my surgical colleagues: Dear friends, We never send one of our ICU patients to the OR without making sure you know who the patient is, why they're going, and what needs to be done Please don't send a postoperative patient from the OR to the ICU without telling us What did you find? What did you do? What went right (or wrong!)? What are you worried about? What do you want us to do next? What do you expect? We don't need a 20-minute presentation. We need 2 minutes of your time. Or just a text/message! A handoff isn't just courtesy. It's patient care Please talk to us #MedTwitter #ICU #FOAMcc

Professional athletes and sports psychologists talk about the value of doing what they do ”for the love of the game”. The idea that personal fulfillment and joy must always remain a core motivating principle to perform at the highest levels. When that gets lost, the execution and precision fall off and performance suffers. Not sure that I see that recognized often enough in what we do as physicians. There are a myriad of distractions and easy sources of frustration and angst. We forget that pure joy we had when we decided that surgery was the calling. I had a resident tell me today the thing he enjoys most working in my room is that there is that joy and “love of the game.” That’s gonna stick with me. And I’m grateful to have heard that.

I’m not much of a scripted TV show fan. So I have been in the dark for 6 years to the brilliant of #tedlasso I admit my failing. This show is better than just about any leadership/team building course offered. I’ll fight over this. It’s fucking brilliant. piped.video/3S16b-x5mRA?is=pb_q…
