I saw something at work recently that stayed with me. A young man came into the hospital looking very uncomfortable. He had been having abdominal pain for almost two days. At first, he wasn't particularly worried. He told me, “Doctor, I think it’s just malaria. I’ve been taking drugs but it hasn’t really stopped.” I asked him a few questions and examined him. Something didn't quite sit right with me. His pain had started around his middle abdomen but had gradually moved to the right lower side. He also had poor appetite and had vomited a few times. I explained that we needed to run some tests and also an ultrasound. He immediately asked: “Doctor, how much will everything cost?” That question is very common in our hospitals. Sometimes, patients aren't refusing treatment because they don't care about their health. They are simply trying to figure out how to survive the bill. I told him we would start with the most important things and avoid unnecessary expenses. We eventually got the investigations done. The result showed that he had acute appendicitis. I explained that he would likely need surgery. He became quiet. Then he said something I'll never forget: “Doctor, I don't have the money for surgery.” He wasn't being difficult. He genuinely didn't know where the money would come from. We discussed the options with him and his family. They started making calls. Someone contacted a relative. Another person contributed what they could. Eventually, they were able to raise enough to get him treated. The surgery went well. A few days later, I saw him walking around the ward, looking much better. He smiled and said: “Doctor, thank you. I thought it was just malaria.” That statement made me think. In Nigeria, we often attribute almost every symptom to malaria. Headache? Malaria. Body pain? Malaria. Abdominal pain? Malaria. Weakness? Malaria. Sometimes it actually is malaria. But sometimes, it isn't. And the danger is that repeatedly treating ourselves based on assumptions can delay the diagnosis of something more serious. The other thing that stayed with me was his struggle to pay for treatment. As doctors, we sometimes become so focused on the diagnosis, prescription and procedure that we forget there is a human being on the other side of the case who may be worried about how to afford everything. That day reminded me that being a good doctor isn't only about knowing what disease the patient has. Sometimes, it's also about listening, explaining things clearly and understanding the reality the patient is dealing with. And yes, sometimes… That simple “it's probably malaria” may be hiding something completely different.
