I think I found the residency résumé industry. Mount Sinai lists Dr. Ratesh Khillan as a voluntary faculty physician/Assistant Clinical Professor affiliated with Beth Israel and Mount Sinai Brooklyn. Meanwhile, his private Brooklyn USCE business advertises Mount Sinai among its “teaching affiliates” while selling clinical rotations and residency-related services to IMGs. His Brooklyn USCE business currently advertises: $1,550: 4-week clinical rotation +$399: opportunity to participate in research with Dr. Khillan LORs on hospital letterhead for some rotations plus CV review, interview prep, residency signaling guidance and other Match services in larger packages. Then I started looking at the research. Two completely unrelated single-patient case reports, published two months apart, have the exact same 10 Brooklyn Cancer Care authors, just rearranged. One is a liver hematoma case and the other is Stage IV endometrial cancer. And this isn’t even the only company selling residency research credentials. USMLE Wise openly advertises $300 “co-author” placement on an active research project and $800 “lead-author” placement. They say participants perform substantive work and meet authorship standards, but those are literally the products being sold to residency applicants. So foreign medical graduates can now shop for American clinical rotations, research opportunities, publication credits, hospital-letterhead recommendations, interview coaching and Match strategy. Meanwhile we’re told a publication-heavy CV proves who the “best” residency candidate is. How much of the American residency meritocracy is actually a credential marketplace?

There is no 'developing' world or 'developed' world. Every country's development is the current maximum potential based on its people and their ability to apply knowledge in all things: government, construction, institutions, planning, law, etc. The 'developing' world can have all the resources and technology as every 'developed' country, but the results will never be the same. Culture and intellect are the driving forces in whatever sense you consider the word 'developed'. This premise is the opposite of modern sociology since at least the 1960s when sociologists began to uncritically believe that all cultures are equal. Read a sociology book from the 1930s, then read Ian Morris or Jared Diamond. Big tone shift. Grammar changes from the objective based on observation to the conditional based on untested hypotheses i.e. 'bad location' or 'lucky latitudes'. The modern sociologist's premise on why outcomes are unequal hang on the subordinator 'if only'. As in, 'if only _____ happened then this culture would be exploring the galaxies'. 'If only the Mongols didn't sack Baghdad then the Islamic world would be dominating technology today' - a (lightly hyperbolized) claim among many sociologists. Which is an odd claim because an equivalent city to Baghdad was Constantinople, and Constantinople was sacked (twice) and eventually evacuated of its native residents...but the West dominates technology today. Also as if technology is the only value of what makes a place worth living in. It is also clear based on light humor in editorial commentary from the pre 1960s books that those authors viewed themselves as equals to the reader. Modern sociologists demand your utmost respect because they wrote the book, and take themselves very seriously.

What older generations fail to grasp but younger ones do is this: the borders of a nation are borders of where a designated people live. When Americans complain about such and such group coming here and saying ‘they’re turning X place into their home country’, that’s because a people are their nation. For example, India is a nation of people. Indians in Frisco, TX will inherently turn it into India because they ARE India. Ditto Mexicans in California. And so on. The land borders inherently mean nothing other than that’s the currently agreed upon designated area for a people to live. To assume any non Western immigrant will immediately adopt American values and etiquette is unfounded, and reveals a lack of pattern recognition or logical faculties. Older generations still think immigrants want the ‘melting pot’ narrative for themselves. This is the disconnect. That might have been true for immigration up to 2001. We do not see that any longer.
