#HFpEF pearl of the day: If a patient with HF symptoms and normal LVEF has E/A ratio < 1 and PA systolic pressure > 50 mmHg on echo, it’s more likely PAH, not HFpEF. Low E/A ratio is due to underfilled LV due to pulmonary vascular obstruction in setting of PAH.

I worked the night shift at Sinai Health System last night. Dr. Paul Koblic worked the casino shift from 9 pm to 4 am. I'm old and he's young. He offered to stay all night if necessary to do the intubations so I wouldn't have to risk getting #COVID19. I'm crying as I write this.

(1/4) 80s woman, DM, multiple comorbidities. EF 25%, persistent CHF and NSTEMI with chest pain with minimal exertion. Severe multi-vessel disease - turned down for CABG. I had a hard time deciding to proceed to PCI with diffuse disease, multiple comorbidities, and frailty.

Mid 70s man, low risk for SAVR. 730 annulus, dagger of calcium in LVOT, very calcified annulus, valve. Options for us are surgery, oversized 29 S3 or Evolut R 34 plus postdil. He wants TAVR. Want to make sure I'm not crazy for pushing surgery here. Posted w/ permission. #TAVR
