I can only begin to imagine how sad and triggering this new (déjà vu) report is for parents and families like yourself.
Culture change and addressing teamwork is needed in so many places, not just in UK.
But the themes in today’s report - poor teamwork, a lack of kindness & compassion, a lack of honestly when things go wrong & an institutionalised culture of denial & reputation management - have all been identified many many times before.

I think if you listen to trainees, you will learn a huge amount about where is providing safe care, and where isn't. Do we need more reports and Investigations or do we need to use the information we already have, to make changes before more tragedies occur?
Devastating for all involved. My thoughts are with all those families impacted and all those committed to safe maternity and newborn care. Much to be done. I’m genuinely curious about the Trust by Trust approach in UK versus a National or system wide review.
