Recommending the COVID shot to immunocompromised adults and to children was a policy decision, not a conclusion forced by solid evidence that it would protect them. Treating that recommendation as settled medical advice was unethical. Infection was not durably stopped in any age group once variants arrived and immunity waned. The claim that the shot reliably lessens symptoms is thinner still where the immune response is blunted, or where the baseline risk was already small. Pfizer’s and Moderna’s Phase 3 trials excluded most people with serious immunosuppression. They supply no direct placebo-controlled evidence of fewer severe cases in transplant recipients, people on B-cell depleting drugs, or others with major immune defects. The advice rested on higher baseline risk plus smaller antibody and T-cell studies that often showed weaker responses than in healthy adults. That is an extrapolation, not the original trial result. Children were at very low risk of severe COVID. Early pediatric authorizations leaned on antibody bridging and smaller safety datasets, not on large trials powered to show fewer severe outcomes. Pushing the product onto the highest-risk and the lowest-risk groups without those trials was policy first. It was not evidence that the shot had been shown to help them.
