Back when my lab did a lot of ChIP-seq, we spent a lot of time validating our antibodies. Roughly half were bad. Now we know why.

When you intubate a 23-weeker in the delivery room, and the baby sits comfortably on 21 percent, do you still give surfactant? In the episode we released on Monday, Roger Soll flips his own script. He has spent his career talking about intubating to administer surfactant. Here he argues the reverse. If you intubated that baby because they needed respiratory support, then that baby needs surfactant, and the FiO2 is not what decides it. The mode of ventilation trumps the oxygen requirement. If the tube is in, the surfactant should follow. He knows this is contested and says so plainly. But it puts pressure on the small habits many of us default to: confirm the tube, get up to the unit, settle the baby, wait and see whether the oxygen climbs. Soll calls that delay a mistake. This is indeed a big problem in Pediatric Residency Education. Only 13% & 44% making competency on flow-inflating bag and T-piece respectively. Ventilation skill is critical to neonatal resuscitation. Cuts landed where women lead: breast, ovarian, and gynecologic cancers.
