After the shooting, the reviewers tried to interview Harpole and Flood by phone, email, and certified letter. Flood did not respond. The report still concludes UVU staff did not act in bad faith. 1.) Policies existed; they weren’t used. Policy 425 / MEAC is supposed to force a security assessment. Event was formally approved Aug. 27 before any written assessment existed. 2.  UVU deferred to TPUSA on venue and tickets even after staff said the courtyard was hard to control. 3.  No joint walk-through with TPUSA security (Flood / Harpole). Reviewers called that a critical miss. Flood, Harpole, Crofts, and the UVU TPUSA club president did not sit for interviews. 4.  Open campus, no perimeter. No fencing, no screening, no credentials. Only bike racks near the canopy; the crowd knocked them down after the shot. People walked into Fugal and tried to use the president’s balcony. 5.  Police department is tiny. Report says 13 sworn is not enough for UVU’s size. Chief sat too many layers below the president (they’ve since moved him under VP Peterson). 6.  No mutual aid requested before the event, even after the crowd was obviously larger than 600. Orem PD/Fire/DPS/FBI response after the shot is one of the few things the report praises. 7.  No dedicated protective-intelligence workup on the speaker/event beyond SIAC check and social listening. An anonymous threat the morning of was called in to SIAC and deemed unsubstantiated.
