Highly recommend this letter to the editor from van As, Tree and Ost on the HEAT/ICE focal HIFU/cryotherapy registry paper in Eur Urol. Much of the media coverage emphasized fewer side effects than surgery or RT. Yet this paper reports no functional, QoL, or toxicity outcomes. Preserving urinary and sexual function is central to the rationale for focal therapy, so that comparative claim goes beyond what this analysis can show. Retreatment matters for that question. The 10-year cumulative incidence was 34% for a second focal ablation (not counted in the paper’s local retreatment endpoint) and 30% for radical treatment. These outcomes overlap and should not be added, but they highlight why function needs to be assessed across the full course of care, including the potential effects of subsequent treatment. Median follow-up was 3.3 years, with 173 men (5%) followed to 10 years. These registries are valuable, and focal therapy deserves rigorous study. A side-effect advantage was not demonstrated in this analysis, and I’d be cautious about the paper’s suggestion of first-line use alongside surgery or RT. HEAT has previously reported function after a second HIFU treatment (Lovegrove, BJU Int 2020). Are there plans to extend that work with validated PROs across the full course of care, including retreatment and salvage? That would be a valuable next step toward the comparative evidence patients need. #PCSM #RadOnc
