The most expensive part of a hospital bill may never touch the hospital at all. A patient admitted for care has no way of knowing whether the price on a medical consumable reflects its actual cost or a markup fixed long before it ever reached the ward. That gap in information is, at its core, a public health issue. A survey of hospital consumables in Maharashtra found an IV infusion set with a trade price of ₹11.05 carrying a printed MRP of ₹325 ; a markup of 2,841%. A syringe procured at ₹6.75 carried an MRP of ₹57.20. A catheter procured at ₹29.41 carried an MRP of ₹310. These are not elective purchases. Patients cannot compare prices, seek alternatives, or question a number printed on a box while receiving care and the MRP itself is often fixed upstream by manufacturers and distributors, disconnected from the trade price by a wide, unexplained margin. The result is a system where the party bearing the cost has the least information to evaluate it. The regulatory gap is structural: scheduled medicines are capped under the Drugs (Prices Control) Order, 2013. Most medical devices and consumables are not leaving both the pricing and the information around it almost entirely unmonitored. Here are few of his visionary enactments: 1) The Assam accord of 1985 - The one that took place between the govt and the leaders from Assam movement that lasted for 6 long years. 2) The Mizoram accord of 1986 - The one that saw a peaceful transition of power from an insurgent leader to become CM; notably the then Congress CM stepped down to make it happen. 3) The 1987 Indo-Srilanka accord - The only accord between India and Sri Lanka till date in terms of the Tamil rights. 4) Operation cactus of 1988 - The one in which India successfully thwarted the coup attempt in Maldives. 5) Voting rights at the age of 18 - Arguably, the most historic one. The 61st constitutional amendment reduced the voting age from 21 to 18. There are many more such as the Rajiv-Longwal accord, Ganga action plan, Tripura accord, anti-defection law to name a few. Basically, between 1985-88, in a span of 3 years, Rajiv's govt had explored almost every avenue of political accommodations available to bring peace and solve problems. The Ex-IAS officer was right, man really wanted to solve all the world problems. I even have a theory that had Rajiv not been assassinated, BJP wouldn't have formed govt in the late 90s. Which agency refunded? Were refunds done to evade accountability for faulty equipment? -Why have accompanying notes to the audit report not been uploaded? A fund set up to help citizens during disasters, which has accumulated thousands of crores (including from foreign sources), is shrouded in secrecy. Still no information available on sources of funds. The PMCARES Fund continues to evade public scrutiny by refusing to submit itself to the RTI Act.
