Enucleation is only if tissue is enucleated not resected ! This a good surgical video to demonstrate this . And if one leaves such tissue patinet will come back .

Early apical release is not one deep cut. It is a stepwise dissection: preserve the sphincteric mucosa, advance layer by layer, and read the tissue. What’s going on in some centers? 🤔 A patient with a history of Rezūm and TURP is still voiding terribly, despite alfuzosin + finasteride. And now, as a third step, he’s being offered prostatic artery embolization (PAE). 😲 What about #HoLEP? Why not? We should offer patients what they need—not just what we have or what we know how to do. 😀
