Please check out our review on @ACG_EBGI 🔔🔔⬇️ I am very grateful to Dr. @BilalMohammadMD for this opportunity. “ESD vs TAMIS for Early Rectal Neoplasms 📜📋Question: Is endoscopic submucosal dissection (ESD) noninferior to transanal minimally invasive surgery (TAMIS) for preventing local recurrence after local resection of early rectal neoplasms? ❇️✂️✂️🔪Our Practice: Patient selection and local expertise are key factors when choosing between ESD and TAMIS. Large rectal neoplasms need special consideration because of the risk of submucosal invasion. In addition, rectal surgeries are associated with higher morbidity as compared to colon surgeries.

Underwater EMR of a circumferential periappendiceal polyp extending into the appendiceal orifice. Key technical lessons: ▪ Be patient with underwater visualization—continue irrigating until the cecum is completely flooded. ▪ Adequate immersion allows the intraluminal appendiceal component to float into view, revealing the true extent of the lesion. ▪ Anchor the snare tip beyond the lesion, then gradually open the snare to capture as much tissue as safely possible. ▪ Before transection, confirm that the closed snare includes a rim of normal mucosa beyond the lesion margin. ▪ Carefully inspect the appendiceal lumen after resection to exclude residual adenomatous tissue. ▪ When closing the defect, preserve the appendiceal orifice and avoid clipping it shut to reduce the risk of delayed appendicitis. Key message: successful underwater EMR requires patience, complete immersion, deliberate snare positioning, and preservation of appendiceal drainage.

One of the biggest misconceptions about professional coaching is that it’s only for when something is wrong. In medicine, we readily encourage mentors, sponsors, and teachers. We should normalize coaches too. The best time to invest in coaching is often before burnout, conflict, or career uncertainty. Early in your career, coaching can help you clarify your values, develop your leadership style, navigate difficult decisions, and build a career that reflects who you want to become.
