Purchasing, entering old chart info, entering current and constant chart info, costs a lot and takes a lot of time and money. The consolidation of medical clinics into hospitals is also because these costs were easy for big companies but absolutely death to a little practice.
So your only choice is to go to a big box clinic and get churned through the system and digitized and treated like shit.
Telling an eighty-year-old woman to "check the portal" for serious biopsy results is cruelty disguised as digital efficiency.
