Roughly… Medicare pays about 20cents on the dollar, so providers charge $5 so they recoup their efforts. So everyone else needs insurance… to pay massive monthly premiums to insurance companies… to get your $5 bill negotiated nearer to $1… and then charge a 20% deductible —$1. Everything government does to save you money costs you more.

Starts from $5.28T in 2024 U.S. health spending, adds extra care for the uninsured/underinsured, then subtracts assumed cuts in drug prices, admin costs, fraud, ER use, and provider payments. Result: ~$1.04T lower national spending and ~114k fewer deaths. Why it isn’t proof: the authors don’t model provider reactions to Medicare rates, transition costs, or extra utilization. “System spending falls $1T” is not “the federal budget shrinks $1T.” Taxes would replace premiums. Treat it as an optimistic projection, not a settled finding.
