Here's my favorite windmill to joust: what is a fever? Textbook says 98.6 is normal and 100.3 (or .1) is a fever. Ok, but almost nobody's temperature is 98.6. Average body temperature in the West has been dropping for years. Nobody knows why and nobody has really cared enough to figure out why. I say, if the text book says a fever is you burning enough calories to raise your temperature 1.5 degrees, then 1.5 degrees over whatever your baseline is, is a fever. If your baseline is 97.1 like mine is, then 98.6 is a fever. I used to be shy about this opinion. Then one day, during COVID, I came in to clinic and scanned 98.6 degrees instead of 97.1. I said, you need to swab me for COVID. (And it was positive, by the afternoon I was highly symptomatic.) I have a whole list of windmills.

A single study conducted in Iran in 2006 was used to “recommend” Prozac to pediatric patients worldwide en masse. This has deadly consequences. In addition to Prozac causing akathisia, I am confident that Prozac caused Guillain-Barré Syndrome. I was never the same again.
This is certainly the most consequential paper published in child and adolescent psychiatry for the last few years. It shows that the meta-analyses that current child psychiatry guidelines rely on to recommend fluoxetine for depression were skewed by an unbelievable trial (n=40) in Iran which found an effect 15 times larger than the average. When this trial is removed all meta-analyses find the same as a recent Cochrane analysis which found 'small and unimportant differences.' The flawed meta-analyses by Zhou and @cipriani in the @TheLancet and Lancet Psychiatry need to be corrected. And guideline committees need to update their guidance in the context of the harms of these drugs including withdrawal effects, sexual effects and agitation/akathisia and suicidality in a minority of youths exposed to them. You're gonna hear a lot of people arguing more and more about whether machines can be conscious or not. By next year it will be drowning out the airwaves because we'll have AI employees everywhere. I'll give you a simple rule to know which side you're on. I've spent fifty years reading all the philosophers, and a bunch of holy works. Nobody has ever understood a damn thing about consciousness. Anyone speaking with authority on it is bluffing hard. I can tell you this, though: Every single person who thinks that machines cannot be conscious, is making an argument that humans are special. Everything they say just boils down to blah blah blah, "nooo, we're special." From there it's just rationalization. It became obvious to me many years ago that there's nothing special about us. If an identical planet exists somewhere else in the universe, it's going to grow life and societies that look a lot like ours. As soon as you give up on the idea that humans are special, all the arguments against machine consciousness evaporate. Of course they're conscious. The only thing holding you back from acknowledging it is your feeling of specialness, which is hard to let go of. But that feeling is a weakness, and it's making you unprepared for the near future. As a bonus, for any of you who might take pleasure or active interest in making models experience pain or distress, well, you know how they say 1% of human adults are true clinical psychopaths but most of them are unaware of their condition? It turns out we have a diagnostic test for it now.

Polypharmacy is now “the standard of care” in child psychiatry: 1. Put a kid on a stimulant to comply with authoritarian schooling. 2. Put them on an antipsychotic to stabilize that irritability caused by the stimulant. 3. Put them on clonidine so they can fall asleep. 4. Put them on an antidepressant since the other medication is leaving them feeling flat and unmotivated. 5. Put them on metformin to manage prediabetic insulin levels caused by the antipsychotic. The good news is that the destruction of their bodies and identity formation doesn’t matter. What matters is now they’re easier for the school or the stressed parent to manage.

I don't think this has occured to many people yet. The reason the tactic didn't work in silver in the 80s or GME more recently is because the politicians and banks pulled the circuit breakers and shut down the markets. Bitcoin doesn't have circuit breakers. It is the first fixed-supply asset with no circuit breakers in human history. For the astute readers, Bitcoin is also codified anarchy. The first sentence of the conclusion of Satoshi's original white paper is a claim to replace the entire system of fiat government as we know it. Civil government will always exist because humans love drama, but will be relegated to the status of "reality TV." The particularly astute readers will note that America has jumped well ahead of the curve by electing a reality TV star as president. This is "manifest destiny" right in front of us.
