At Worthing my Clinical Director, a Consultant in A&E, offered me a Physicians Associate because she knew I liked innovation. She said 'The PA will go to A&E and clerk patients so your Residents can do Discharge Summaries" I refused the offer.

- 19yo with disorder that makes him prone to blood clots - ⁠calls an ambulance with symptoms typical of a blood clot in the lungs - ⁠blood test to rule out blood clot not performed - ⁠discharged with antibiotics - ⁠reattended A&E later same day coughing up blood (red flag for blood clot). Discharged again. - ⁠saw GP a week later who sent to A&E as had obvious symptoms of blood clot - ⁠discharged with no investigation for blood clots from A&E again as doctor “forgot” to refer to be admitted for tests - ⁠attended later same day as symptoms worsening. WAS actually admitted but communication so poor and kept in waiting room on a chair after “admission” that he didn’t realise he’d been admitted and went home to sleep. - ⁠died next day at home His life would have been saved with a simple blood test at any of the FIVE separate encounters with medical professionals.

Also wouldn’t we be extremely worried if an ED Senior Resident Doctor (presumably they mean Tier 4 or 5 level Senior Dr) needs reminders for this level of basics? This is one of @EastCheshireNHS responses for the PFD- these posters 😬

I have a new diagnosis given without ever seeing a doctor. I have been prescribed medication that says do not stop taking it without consulting a doctor. Actually, I’d really like to consult a doctor before I start taking it ‼️ #NHSNoReply

Advanced practice deserves to be an advancement of our colleagues' base professions, using their valuable expertise. There are many longstanding examples of this working well. Doctor substitution is unsafe and unfair on everyone - patients and staff. It must end.
Revealed: the unsafe doctor substitutions putting patients at risk. We need NHS leadership to wake up. Hospitals knowingly using doctor substitutes, following years of shambolic workforce planning by successive governments, is a serious threat to patient safety. We are calling for a prohibition on the use of non-doctors on doctors’ rotas and an urgent review of advanced practitioners’ scopes of practice.
