On this #WeekinOncology #Melanoma by OncoAlert🚨, our #OncoAlertAF colleagues Dr. Mario Balsa 🇪🇸 discusses an article out on Nature communications and how a melanoma study reveals the devastating long-term impact of false-negative diagnoses—and why getting the diagnosis right the first time matters. Would a 2-year diagnostic delay change the outcome?

A large German 🇩🇪 study of 9,063 melanoma patients found that an initially false-negative cancer diagnosis can have serious long-term consequences. Among 206 patients with a missed diagnosis, the median delay to correct diagnosis was 24 months. Their 10-year recurrence-free and melanoma-specific survival were significantly lower than those diagnosed correctly.
✅️ Most strikingly, an initial false-negative diagnosis was associated with an average loss of 8.2 years of life, highlighting the critical importance of accurate and timely cancer detection.

Effect of Screening with Multicancer Early-Detection Test on Late-Stage Cancer Diagnosis
A large randomized English🇬🇧 trial involving 142,250 adults aged 50–77 evaluated annual multicancer early-detection blood testing alongside usual care.
➡️ After three screening rounds, there was no significant reduction in stage III/IV cancers compared with usual care alone (incidence rate ratio 1.03; P=.63).
➡️ Stage IV cancer incidence was lower with screening (IRR 0.86), although the result requires further follow-up. Trial-related adverse events were uncommon and nonserious.

The OlympiA trial shows that one year of adjuvant olaparib provides sustained benefits in patients with high-risk, HER2-negative early breast cancer and germline BRCA1/2 pathogenic variants.
➡️At 6.1 years, olaparib improved invasive disease-free, distant disease-free, and overall survival, with a 4.4% absolute six-year OS benefit.
➡️Benefits were consistent across subgroups. Toxicity remained acceptable, with no increased risk of MDS or AML, supporting continued adjuvant use.
