#ISLCPAIS2026 abstract review. My thoughts on abstract 2 One of my fav studies which basically replicates @VirusesImmunity bigger studies in transfer of IgG from patients with #longcovid to mice. This study provides further evidence for an immune-mediated mechanism of pain in Long COVID, showing that patient-derived IgG targets peripheral sensory neurons in vivo and ex vivo. These findings strengthen the rationale for testing immunomodulatory approaches such as IVIG in carefully designed clinical trials. Importantly, funding strategies should not focus exclusively on viral persistence: the growing evidence for pathogenic autoantibodies and immune-mediated neuronal dysfunction makes immunomodulation a parallel, high-priority therapeutic avenue that warrants properly powered clinical trials. I’m surprised this is still not on the priority list

How does vaccination help beyond antibodies? Vaccinated individuals had higher tissue-resident memory T cell (TRM) signatures in the nasal mucosa and mounted Type II IFN responses earlier. This points to mucosal TRM as a key mechanism — and a target for next-gen vaccines. (5/)

Implication: if we want vaccines that limit transmission, not just disease, we may need to seed TRM in the nose. Current intramuscular vaccines do this to some extent, but poorly. Prime & Spike strategies do this very well. As always, very grateful to all who contributed 🙏🏼
