I think @IyerGopa @DrRosenbergMSK are looking into this more. In our recent paper in @CellReports we explored a patient with serial cfDNA on FGFRi who developed resistance and using paralogy analysis found resistance mut (Suppl Fig 5)

This led to evaluating such patients with matched primary/metastatic tumors with plasma cfDNA and found that 20% of targetable mutations were only seen in the cfDNA analysis. Suggesting that tumor sequencing underestimate the discordance of alterations in bladder cancer

We enlarged our matched primary-metastatic cohort (n=119) with NGS MSK-IMPACT seq and found 23% of actionable alterations were discordant! This suggested that primary archival tumor tissue may not be sufficient to identify such alterations

To determine if mutation rates were reflective of timing during tumorigenesis, we performed WES of matched primary-metastatic tumors. We found early branched evolution with distinct discordances, notably ARID1A only within the metastatic tumors when discordant
