Great first year for the Research Update in Neuroscience for Neurosurgeons (#RUNN) course at Asilomar in Pacific Grove, CA, after many years at Woods Hole, MA. Â A full house of neurosurgery trainees with great questions.
I had the chance to talk about convective delivery for nervous system gene therapy. The residents and fellows in that room are the ones who will carry this from clinical trials into everyday practice. With trainees like these, the future of the field is bright.

Evidence-based guidance on which germline genes to include on a panel (AIP, MEN1, CDKN1B, PRKAR1A, SDHx, MAX, GPR101, GNAS), who to test, how to counsel and interpret results, and when somatic testing is worth considering in aggressive PAs.
Key takeaway: genetic testing is now a real adjunct in pituitary care — best delivered by a multidisciplinary team with endocrine, genetics, and lab expertise.

Our multicenter international study of 162 patients with Cushing’s disease shows that endoscopic endonasal surgery achieves excellent resection + durable disease control. Early postop cortisol is a powerful predictor of long-term outcome.
▪️ Gross-total resection in 86.4% of cases — rising to 91.9% in tumors without cavernous sinus invasion
▪️ Hormonal remission in 78.4% at follow-up 29.8 months
▪️ POD1 cortisol <2 µg/dL predicted a 91.8% recurrence-free survival rate
▪️ No visual or vascular complications + no mortalities across the series

Neurosurgeons from different parts of the world deal differently with complications:
- Our European colleagues were more likely to question their clinical abilities and less likely to admit fault to the patient every time
- US neurosurgeons were more frequently implicated in litigation and were more likely to utilize faith, mindfulness, and therapy
- The principle of forgive and remember provides a constructive framework facilitating moving forward, while remembering and learning from complications. Great work by our brilliant research fellow @VictorGabriel_H@TJUHNeurosurg, having our artwork on the cover of the journal was the icing on the cake! @TheJNS
